REGISTERED NURSE – Aged Care & NDIS / Community Nursing

The Registered Nurse – Aged Care & NDIS / Community Nursing is responsible for providing safe, high-quality, person-centred and evidence-based nursing care to older people receiving aged care and Support at Home services, and participants receiving supports under the National Disability Insurance Scheme (NDIS). The Registered Nurse supports clients and participants to maintain their health, safety, independence, dignity, choice and quality of life while receiving care in their home and community.

The role provides clinical assessment, nursing care, care planning, clinical monitoring, risk management, health education and clinical oversight within the Registered Nurse's professional scope of practice. The Registered Nurse is also expected to contribute to clinical governance, incident management, infection prevention and control, quality improvement and the secure delivery of services across both Aged Care and NDIS programs. Please note that this is a part-time 10 hours a week role predominately working from home. You will need to travel if and when required to the aged care facility or participants home.

Key responsibilities
n
Provide person-centred nursing care within professional scope of practice
n
Conduct comprehensive nursing and clinical assessments
n
Develop, implement and review clinical and nursing care plans
n
Identify clinical risks and establish appropriate risk-management strategies
n
Monitor clients and participants for changes in health status or deterioration
n
Escalate clinical concerns appropriately
n
Provide nursing interventions based on assessed needs
n
Maintain accurate and timely clinical documentation
n
Work collaboratively with clients, participants, families,



representatives and health professionals
n
Provide clinical guidance to support workers where appropriate
n
About you
n
Current registration as a Registered Nurse with the Australian Health Practitioner Regulation Agency (AHPRA)
n
Division 1 Nurse
n
Nursing qualifications recognised in Australia
n
Current AHPRA registration without conditions that prevent performance of the position
n
Current National Police Check as required
n
NDIS Worker Screening Check where required
n
Current First Aid and CPR certification where required
n
At least 3 years' experience as a Registered Nurse in both Aged Care and NDIS/disability services is an essential requirement
n
3 years of practical nursing experience working with older people receiving aged care services
n
Practical nursing experience working with people with disability receiving NDIS or disability supports
n
