Job Description
The Clinical Nurse Specialist – Discharge Planner at Department of Health in Tasmania operates within the MHIU of the Royal Hobart Hospital to support secure, timely discharges from an acute inpatient setting.

Working as part of a multidisciplinary team, you will coordinate with treating clinicians, NDIS providers, and accommodation services to ensure recovery‐oriented, least restrictive care transitions.
