Job Description
Warrigal is seeking a Care Partner to coordinate and manage the care needs of clients living in the community.
You will work with the Care Services Manager and Home Services team to align services with care plans and approved frameworks.
You will undertake comprehensive assessments, develop and review care plans, and balance multiple client caseloads while focusing on outcomes and wellbeing.
Collaboration with clients, families and providers is essential to minimise disruptions and support
