Job Description
Justice Health & Forensic Mental Health Network is seeking a Clinical Nurse Specialist in Primary Care to deliver care to adult patients with acute or chronic illnesses in the custodial setting.
You will support Justice Health NSW clinicians to ensure continuity of care, case management and transfer of care into the community, with post-release follow-up as required, and take accountability for planning, development, implementation and evaluation of primary care services.
