Launch Housing is seeking a dedicated Northern Outreach Case Manager to support people experiencing or at risk of homelessness. You will deliver assertive outreach, build therapeutic relationships and provide practical, recovery-based planning to clients in Collingwood and surrounding areas.

The role requires tertiary qualification or demonstrated social services experience, solid communication and team-oriented skills, and a commitment to ending homelessness.
