Palm Lake Care is seeking a Clinical Coordinator - Admission & Assessments to lead foundational assessments and ensure quality documentation of resident needs. You will develop care plans, coordinate ANACC reviews, and manage funding processes with a focus on resident outcomes and organisational KPIs.

The role requires current AHPRA nursing registration, solid leadership, and expert knowledge of ANACC and aged care policies.
