An experienced Accredited Practising Dietitian (APD) role that pairs remote case conferencing with hands-on clinic work, 1-2 days per week. The hybrid model combines structured multidisciplinary case conferences with in-clinic dietetic consultations, offering a more structured, collaborative approach to chronic disease management with clinical variety.

Key responsibilities

Contribute to remote multidisciplinary case conferences
n
Develop and guide nutrition interventions for chronic conditions
n
Provide evidence-based dietary recommendations
n
Deliver in-clinic consults supporting care plan implementation
n
Maintain high-quality clinical documentation and reporting
n
Review patient histories and contribute to nutrition-focused care plans
n
Provide education and follow-up aligned with care outcomes
n

About you
n
n
Accredited Practising Dietitian (APD), registered with Dietitians Australia
n
Confident working remotely and independently
n
Strong communicator, comfortable collaborating virtually
n
Person-centred, empathetic approach to care
n
Experienced with telehealth/hybrid service delivery
n
Solid documentation and time-management skills
n
Interest or experience in chronic disease management
n
n
$100–$125/hr, with robust earning efficiency across the day
n
