The Sydney Children’s Hospitals Network in Current South Wales is seeking a Care Coordinator to support Aboriginal children with chronic conditions in the community, linking families to primary health services and multidisciplinary care. You will work within the Aboriginal Chronic Care Team (ACCT) and partner with the La Perouse Family Support Team (FST).
The role includes clinical supervision with prospects for ongoing education and training.
