Rise is looking for a Case Worker to support young people across the City of Swan and Shire of Mundaring through outreach support, case management and youth engagement programs. This is a hands‐on role where you'll manage a caseload of young people while also leading activities and programs at the Stratton Edge Youth Centre. This is a fixed-term position until 30 June **** based across our
Helen's Place Office (Middle Swan), the City of Swan and Stratton Youth Centre
, working Mondays and Wednesdays (9:00am to 5pm some flexibility) and 11:00am-7:00pm shift on Tuesdays, Thursdays and Fridays.
What You'll Be Doing
Providing case management support to young people experiencing a range of challenges
Developing, implementing and reviewing support plans
Managing an outreach caseload across the City of Swan and Shire of Mundaring
Acting as the lead worker at Stratton Edge Youth Centre
Facilitating drop-in sessions, youth activities and school holiday programs
Building and maintaining relationships with schools, community organisations and other support services
Coordinating referrals and linking young people with appropriate supports
Maintaining accurate case notes and records within Rise's client management system
Preparing reports for funding partners including the Department of Communities and City of Swan

Here At Rise, We Nurture Potential, But We Do Need You To Have a Few Tricks Up Your Sleeve Including The Following
A qualification in Community Services (or similar), currently working towards one, or relevant experience in a similar role
Experience supporting young people experiencing complex challenges
A strong understanding of the issues and barriers impacting young people at risk
Well-developed communication and relationship‐building skills, with the ability to engage effectively with young people, families and community stakeholders
Experience coordinating youth activities, events or group programs
The ability to work independently and confidently within a community setting
Solid organisational skills and the ability to manage competing priorities
IT skills, including using Microsoft Office and client management systems
Strong record‐keeping and report‐writing skills, with attention to detail
Formalities (The serious stuff!)
Appointments to this position require a NDIS worker screening check and working with children check. Costs for new checks will be covered and/or reimbursed by Rise.
