This is an exciting role with in City of Whittlesea with a focus on delivering person cantered, goal directed care to live independently meaningful in the community. This role is responsible for developing and recurring annual client care review and working with the client to identify changes in their health, wellbeing and overall needs.
The part you will play
As part of the Case Management team you will work closely with internal teams, stakeholders and members of our community.
Working on:
Clients residents of City of Whittlesea
Carers and family members
Relevant local agencies and service providers
Ageing Well Department Staff
Community Wellbeing Directorate
Other Council Departments and Staff
