Job Description
Melbourne City Mission is seeking a dedicated skilled to support young people transitioning from out-of-home care.
You will provide flexible, holistic coaching, develop individual support plans and work within program guidelines to ensure successful outcomes.
The role requires trauma-informed knowledge, collaboration with community agencies, and strong administrative and reporting skills.
A degree in Youth Work, Social Work or related field is essential.
