As our services continue to grow under Support at Home, we are seeking an experienced Care Partner who understands contemporary aged care and can confidently guide clients and families through the program.
About the role
As a Care Partner, you will be responsible for the ongoing care management and coordination of services for approximately ***** clients.




You will work closely with clients, families, representatives, care workers, allied health professionals, nurses and external providers to ensure each client receives appropriate and well-coordinated support.
You will be supported by a dedicated rostering team while retaining responsibility for overseeing each client's services, care plan, budget, changing needs and overall experience.
The position is based at our Spring Hill office, with regular travel to clients' homes across our service area. Most clients are located throughout Brisbane and Moreton Bay regions.
Key responsibilities
In this role, you will:
Manage an established caseload of Support at Home clients.
Complete client intake, onboarding, assessments and ongoing reviews.
Develop and maintain individualised care plans, budgets and service agreements.
Identify changes in clients' needs, risks, goals and circumstances and coordinate an appropriate response.
Monitor client budgets, expenditure and funding utilisation.
Explain fees, budgets, service options and Support at Home arrangements clearly to clients and families.
Coordinate personal care, domestic assistance, social support, respite and other in-home services.
Arrange access to nursing, allied health, assistive technology and home modifications when required.
Source and coordinate suitable third-party providers and contractors.
Work closely with rostering and service-delivery teams to address service gaps and maintain continuity of care.
Maintain accurate, timely and compliant client records.
Respond professionally to feedback, concerns and complaints.
Contribute to quality improvement and compliance with aged care legislation,



program requirements and the Aged Care Quality Standards.
About you
You will be well suited to this position if you are an experienced aged care professional who can balance strong client relationships with accurate administration, financial oversight and regulatory requirements.
You will ideally have:
Demonstrated experience as a Care Partner, Care Manager, Case Manager or Home Care Coordinator.
Practical experience coordinating Home Care Package or Support at Home services.
A sound understanding of the Support at Home program, including service categories, care management, funding and participant contributions.
Experience completing assessments, care plans, reviews, budgets and service agreements.
Confidence monitoring expenditure and helping clients make informed decisions within their available funding.
Experience coordinating allied health, nursing, assistive technology, home modifications or third-party services.
Strong written communication, documentation and record-keeping skills.
The ability to manage competing priorities and follow matters through to completion.
A calm, respectful and solutions-focused approach when working with clients, families and service providers.
Confidence using client management systems and Microsoft Office applications.
A qualification in aged care, community services, nursing, allied health or another relevant discipline will be highly regarded.
Essential requirements
The successful applicant must have:
A current Australian driver's licence.
A current National Police Check, or willingness to obtain one.
