Ochre Health is seeking a agile Social Worker to join the team at Ochre Medical Centre Bruce in ACT.
The role is part-time (minimum 19 hours per week) and focuses on psychosocial assessments, counselling, and crisis intervention for patients in a busy primary care setting.
You will work with GPs, nurses and administrative staff to deliver holistic care, supporting patients through complex social and health-related challenges and coordinating referrals to external services as needed.
