HammondCare is seeking three part-time Clinical Partner – Registered Nurse (0.6FTE) to cover the Central West/Nepean/Blue Mountains region, including Penrith, Hawkesbury, and Upper Blue Mountains. The role focuses on home-based care, assessing needs, and delivering tailored interventions to help older adults remain at home.
