Key responsibilities
Undertake client intake, assessment and eligibility screening for access to homelessness and housing support services
Develop, implement and review client-centred case plans that identify goals, strengths, support needs and pathways to housing stability
Provide ongoing case management, advocacy, information, referrals and practical support to assist clients in achieving their goals
Undertake risk assessments and respond appropriately to crisis situations in accordance with organisational policies and procedures
Support clients to maintain safety, particularly women and children experiencing domestic and family violence
Assist clients to access specialist services including health, mental health, alcohol and other drugs, legal, financial, education and employment support
Assist clients to secure, establish and maintain safe, affordable and appropriate accommodation
Support clients in their interactions with community housing providers, real estate agents, landlords and other housing services
Promote client safety through risk assessment, safety planning and referrals to appropriate specialist services
Work collaboratively with internal teams, partner organisations and community stakeholders to coordinate services and achieve positive outcomes for clients
Selection Criteria
Certificate III in Community Services, or a related qualification, or demonstrated equivalent experience in the community services sector
Experience working with individuals,



families or vulnerable clients experiencing complex needs, including homelessness, housing instability, domestic and family violence, mental health concerns or other social issues
Demonstrated ability to work independently, manage competing priorities and organise workload effectively
Ability to work collaboratively within a team environment and contribute to positive team outcomes
Strong communication and interpersonal skills, with the ability to engage respectfully and effectively with a diverse range of people
Demonstrated client-focused approach with the ability to build rapport, provide support and respond empathetically to client needs
Demonstrated resilience, sound judgement and the ability to respond appropriately in challenging situations
Working with Children Check
Criminal History Check
Valid Driver's License
