Dhelkaya Health in Victoria is seeking an At Home Nurse to join our At Home Program, delivering high‐quality clinical care to clients in the comfort of their own homes. The role involves developing care plans and coordinating with a multidisciplinary team in a range of home‐based services.
You will provide client‐centred nursing, educate clients and families, and support transitions of care across Hospital in the Home, GEM at Home and Rehabilitation in the Home.
