Anglicare Victoria South Division, Keep Embracing Your Success (KEYS) residential program is seeking a full time Senior Case Manager based in Dandenong to lead intensive case management for young people aged ***** in care.
You will coordinate care teams, prepare KEYS assessment plans, manage case notes and reports, and work in partnership with Monash Health, YSAS, VACCA and DFFH to support transitions to community living while upholding respectful, inclusive practice.
