Just for u Care is seeking a full time Care Partner to provide compassionate, person-centred in-home care to older Australians and people with disability. You will build and maintain care plans, monitor wellbeing, and liaise with healthcare and community stakeholders to optimise client outcomes.
Based in Cheltenham with travel across Melbourne Metropolitan area, Geelong, Bellarine Peninsula and Mornington Peninsula. Immediate start and hybrid work model after probation.
