STAR Community Services is seeking an experienced and passionate CHSP Program Team Leader to provide operational leadership and oversight across our Commonwealth Home Support Programme (CHSP) services.

As the aged care sector transitions to the new Support at Home Program, you will play a pivotal role in leading service excellence, managing change, and helping shape the future of community aged care services.

Reporting to the Client Planning & Service Delivery Manager, you will lead a team of Coordinators responsible for delivering a diverse range of CHSP services that support older people to live independently, safely, and with dignity in their own homes and communities.

About the Role

You will provide leadership, operational oversight, quality assurance, and program management across a range of CHSP-funded services, including:

- Domestic Assistance

- Home Maintenance

- Home Adjustments

- Social Support Group

- Social Support Individual

- Assistance with Care and Housing (ACH) – Hoarding and Squalor

Working closely with Coordinators, support workers, volunteers, contractors, allied health professionals, and community partners,



you will ensure services are delivered in line with CHSP requirements, wellness and reablement principles, STAR's values, and the Strengthened Aged Care Quality Standards.

Key Responsibilities

Leadership & Workforce Development

- Lead, mentor and support CHSP Coordinators to achieve excellent service outcomes.

- Foster a culture of accountability, collaboration, continuous improvement and participant-centred practice.

- Coach and develop teams to build capability across all service streams.

- Lead succession planning, workforce development and capability planning.

- Support organisational readiness for aged care reforms and the Support at Home Program.

Operations & Service Delivery
- Oversee the effective delivery of all CHSP service streams.

- Monitor service performance, participant outcomes and utilisation against program targets.

- Ensure services are responsive, person-centred, safe and outcome-focused.

- Manage relationships with contractors, associated providers and external service providers.

- Identify opportunities for innovation, service enhancement and operational efficiency.

Governance, Quality & Compliance
- Ensure compliance with CHSP guidelines, funding requirements and legislative obligations.

- Oversee service documentation, assessments, care planning, reporting and audit readiness.

- Monitor budgets, funding allocations and contractual obligations.

- Lead continuous improvement initiatives informed by participant feedback and service data.

- Ensure compliance with the Strengthened Aged Care Quality Standards and organisational policies.

Stakeholder Engagement & Strategic Contribution
- Build and maintain strong relationships with My Aged Care, health professionals, community organisations and key stakeholders.

- Support participants experiencing complex challenges, including hoarding and squalor, social isolation and housing vulnerability.

- Contribute to organisational planning,



aged care reform readiness and future service development.

- Support STAR's growth and strategic direction through cutting-edge community aged care solutions.

About You

You are an experienced leader with strong operational, people management, and stakeholder engagement skills.

You will bring:

- A tertiary qualification in Community Services, Aged Care, Social Work, Allied Health, Human Services or a related discipline.

- Demonstrated leadership experience within aged care, community services or a similar sector.

- Strong knowledge of CHSP service delivery, compliance requirements and aged care reforms.

- Experience leading multidisciplinary teams and driving high performance.

- Knowledge of wellness, reablement and person-centred service delivery principles.

- Strong analytical, problem-solving and decision-making skills.

- Excellent communication and relationship management abilities.

- Experience managing service outcomes, budgets, reporting and quality improvement activities.

- The ability to thrive in a dynamic and evolving environment.

Highly Regarded

- Experience preparing services for aged care reform or Support at Home implementation.

- Experience managing Home Adjustments or Home Maintenance programs.

- Experience supporting participants experiencing hoarding and squalor.

- Knowledge of housing, mental health, occupational therapy or allied health sectors.

- Experience managing associated providers, contractors or outsourced service delivery arrangements.

Mandatory Requirements

Successful applicants will be required to hold or obtain:

- Current Driver Licence and access to a reliable, registered and comprehensively insured vehicle.

- NDIS Worker Screening Check.

- NDIS Worker Orientation Module Certificate.

- Current First Aid and CPR Certification.
