The Team

The Adult Mental Health Services North – Continuing Care Team (AMHS N CCT), Launceston is a multidisciplinary community mental health service within the Tasmanian Department of Health. The team provides coordinated, recovery-oriented assessment, treatment and ongoing case management for adults experiencing complex and enduring mental health conditions who require continued specialist community mental health support.

Based at 62–64 Canning Street, Launceston, the CCT works collaboratively with consumers, families and carers, General Practitioners, inpatient mental health services, the Crisis Assessment and Treatment Team, allied health professionals, community organisations, NDIS providers and other government and non-government services to promote continuity of care and support people to remain well and connected within their community.

The multidisciplinary team includes social workers, nurses, occupational therapists, psychologists, Family Carer’s support, Peers Support workforce, medical officers, psychiatrists and other clinical and support professionals. Team members work collaboratively through regular multidisciplinary review, shared care planning, clinical consultation and coordinated case management.

The service has a strong focus on:

- comprehensive mental health assessment and ongoing clinical review;
- recovery-oriented and person-centred care planning;
- psychosocial and functional assessment;
- risk assessment, monitoring and relapse prevention;
- medication and physical-health monitoring in partnership with medical practitioners;
- therapeutic interventions and psychoeducation;
- family and carer engagement;
- coordination with inpatient, primary care and community services;
- supporting safe transitions between hospital and community care; and
- promoting independence, social participation and long-term recovery.

The Role

In this role, you will provide specialist mental health assessment, evidence-based interventions and recovery-focused care to people experiencing significant and complex mental health challenges. You will manage a clinical caseload, working collaboratively with consumers, families and carers to develop treatment and recovery plans that reflect individual needs and goals.
You will work closely with colleagues across a multidisciplinary team, bringing your allied health expertise to clinical decision-making, care planning and continuity of care.

The role offers opportunities to apply and further develop your skills in therapeutic interventions, trauma-informed practice and specialised treatment approaches. You will build strong partnerships with GPs, health services, community providers and other agencies to support coordinated and effective care. As an experienced clinician, you will also contribute to clinical consultation, supervision and the development of students and less experienced staff.

You will be encouraged to grow professionally through clinical supervision, peer review and ongoing professional development, while contributing to quality and service improvement initiatives. This is an opportunity to use your clinical expertise in a role where collaboration,

You will need

- A minimum of two years’ post-graduate clinical experience in an area relevant to mental health service delivery, with demonstrated knowledge and skills in mental health assessment.
- Demonstrated expertise in delivering a range of evidence-based therapeutic interventions for people experiencing significant and complex mental health problems.
- Demonstrated ability to provide recovery-focused case management and trauma-informed care, working collaboratively with consumers, families and carers.
- Strong communication and collaboration skills, with the ability to work effectively within a multidisciplinary team and provide clinical consultation and support.
- Demonstrated experience in liaison and consultation with GPs, health services, community providers and other agencies to support coordinated care and continuity of service.

Details of appointment

Fixed term part time, day worker position, working 64 hours per fortnight, commencing as soon as possible for a period of 12 months.

Salary

$117272 to $126182 per annum (Pro rata). Our Employer 12% superannuation contribution is on top of this amount.

- Salary range is in accordance with Allied Health Professionals Public Sector Unions Wages Agreement No. 2 of 2025.

You’ll have access to salary package a range of benefits including living expenses up to $9,010 + $2,650 in meal entertainment each FBT year + other benefits if eligible.

Allowances

- Please note that access to salaries beyond $123,487 is subject to qualifications and/or application to the personal upgrade scheme

In addition, the following allowances calculated on the salaried incremental point may be available:

- Professional Development Allowance of up to $1200 per annum, paid on a fortnightly, pro rata basis.

Eligibility

Successful applicants will be required to meet the essential criteria.

- Satisfactory completion of an approved allied health professional tertiary qualification/program of study and registered with the relevant National Board or, in the case of self-regulated allied health professions, full membership/eligible for membership with the relevant professional association
- Current Working with Children Registration
- Current Driver’s Licence
