Reporting to the Team Leader, Assessment and Planning, the Peer Support officer organises, delivers and coordinates peer support groups and activities for carers of all ages and backgrounds. The Peer Support officer will regularly liaise with other staff within the Carer Supports program area via working groups with a focus on enhancing peer support related service delivery.
A key pillar of the Department of Social Service’s Carer Action Plan 2024-2027 is the extension of Carer Gateway standard operating hours. This will offer carers the prospect to access all service offerings from 8:00 am until 8:00 pm, Monday to Friday. The Peer Support officer may be required to work a variety of shifts covering the extended operating hours.
Service Description
Holstep Health’s Carer Supports Program supports carers in Victoria, predominantly the Northern and Western Metro regions of Melbourne. Carers can be parents, partners, spouses, young people and children, other family members, friends or neighbours. A carer provides support to someone who needs help managing at home or in the community because they are ageing, living with disability, a mental illness or a chronic medical condition.
We work in partnership with carers to learn about their needs, support them in defining goals and assist with planning for the future. We support carers in their caring roles and aim at maximising carers’ health, wellbeing and social connections.
The Carer Supports Program delivers services through a range of State Government and Commonwealth Government funded programs including the Carer Gateway and the Support for Carers Program.
YOU WILL BE RESPONSIBLE FOR
As a Peer Support Officer, you will;
- Support carers to get connected with and access in person and online peer support
- Develop, plans and facilitate face-to-face and online peer support groups for a variety of carer groups and collaborates with relevant stakeholders to adopt collaborative peer support approaches.
- Plan, organise, co-ordinate and deliver activities that promote and facilitate peer support among Carers using Commonwealth and State Government funding.
- Implement the Carer Gateway In-Person Peer Support Framework, principles and Person Centred Philosophy.
- Promote reflective practice framework that assists in building self- development and self-care strategies with the aim for carers to maintain wellbeing and to have a positive impact on their wellbeing.
- Build and maintain effective relationships with internal and external stakeholders to identify peer support needs, support the development of new peer support groups, and improve access to relevant support.
WHAT YOU NEED TO SUCCEED
- Diploma or Degree in Community Development, Community Services, Social Work or related discipline and relevant experience.
- Proven ability to foster and maintain positive relationships with internal and external stakeholders and to relate to and work effectively with a diverse range of individuals and communities;
- Experience in organising events and facilitating peer support groups and carer activities.
- Working knowledge of the community care sector and experience in a client-facing role
- Very good organisational, planning, analytical and problem-solving skills.
- High level written and verbal communication, negotiation and interpersonal skills and Proficiency in the use of information and communication technologies.
