We're seeking a Disability Support Worker or Nurses to join our team and support a young participant living with a rare disorder that requires intesive support at home. The disorder requires care to support both a physcial and intellectual disability. Seizure management is included in the care requirements.

About the Role:

n

You'll be supporting a complex participant in Narre Warren with a variety of daily tasks and meaningful activities, including:

n

Assisting with appointments, daily routines, and mealtimes

n

Engaging in fun and enriching activities at home and out in the community (swimming, music events, sports)

n

Implementing strategies and care instructions in relation to seizures and positive behaviour support.

n

Key Responsibilities:

n

Deliver person-centred support aligned with NDIS goals

n

Facilitate social and community engagement

n

Provide personal care in a respectful and dignified manner

n

Follow and implement individual support plans,



clinical recommendations and therapy strategies

n

Maintain accurate progress notes and report incidents as required

n

Collaborate with families, team members, and healthcare professionals

n

Qualifications & Requirements:

n

Certificate IV in Disability or equivalent (essential)

n

Experience and/or Education in Seizure Management and/or Epilepsy (Mandatory)

n

Experience with behaviours of concern, de-escalation strategies, implementing behaviour support plans. (Essential)

n

NDIS Worker Screening Check (mandatory)

n

Working With Children Check (mandatory)

n

Experience with intellectual disabilities, ASD, ADHD, OCD, Lissencephaly, Prader-Willi Syndrome, Fragile X, or Angelman Syndrome is highly desirable

n

Strong communication skills and a genuine passion for supporting others

n

Valid drivers lisence, With appropriate car insurance (Essential)
