What You’ll Be Doing Depending on the role, responsibilities may include:

- Managing Support at Home / home care services
- Leading and supporting care teams
- Managing client relationships and family engagement
- Coordinating in-home care and community services
- Developing and reviewing care plans and support plans
- Managing client intake, assessments and service coordination
- Monitoring quality, compliance and client outcomes




- Managing incidents, risks, complaints and feedback
- Working alongside RNs, ENs, PCAs/PCWs, support workers and allied health professionals
- Supporting workforce planning, rostering and staff performance
- Building relationships with clients, families, stakeholders and external providers
- Ensuring services align with Aged Care Quality Standards, legislation and organisational requirements
- Driving continuous improvement and quality care

What We’re Looking For

We’d love to speak with people who have experience in:

Home Care | Support at Home | Aged Care | Community Care | Community Services | Care Management | Case Management | Care Coordination | Client Services | Operations | Team Leadership

Experience with HCP, CHSP, Support at Home, NDIS or community-based care is highly regarded.
