Amity Health is seeking an experienced and passionate Registered Mental Health Nurse to join our Integrated Primary Mental Health Care (IPMHC) team, supporting individuals experiencing mental health challenges across the Wheatbelt region.

This is an exciting opportunity for a senior mental health clinician to provide clinical leadership, care coordination and integrated support for people with moderate to complex mental health needs. Working closely with General Practitioners, mental health professionals and community services, you will play a key role in improving access to holistic, person-centred and recovery-oriented care within regional communities.

About Integrated Primary Mental Health Care (IPMHC)

Funded by the WA Primary Health Alliance, the IPMHC program offers fully funded support for individuals experiencing mental health challenges across the Great Southern and Wheatbelt regions. IPMHC exists to address the significant physical and mental health inequities experienced by people living with mental health issues by improving access to equitable, culturally safe, and integrated care. Grounded in an understanding of the social, economic, and environmental drivers of health inequity, the program delivers flexible, biopsychosocial and collaborative responses that address barriers to care and support early intervention, improved health outcomes, and overall wellbeing.

The program provides counselling services and clinical care coordination through three tailored options:

- Wellbeing Counselling – self-referral for individuals aged 16 and over, offering up to six sessions to enhance mental wellbeing through support, education, and skill development.
- Mental Health Therapy – for individuals aged 18 and over, providing six to ten sessions of comprehensive therapeutic support via GP referral.
- Clinical Care Coordination – assisting GPs in managing patients with severe, persistent mental health conditions, ensuring a holistic, integrated approach to care.

About the Role

This is a senior clinical role responsible for providing mental health nursing leadership, comprehensive assessment, care planning and coordination for individuals with moderate to complex mental health and associated physical health needs.

Working collaboratively with General Practitioners, psychiatrists, allied health professionals, peer workers and community services, you will support integrated responses that address clinical treatment needs, physical health, psychosocial supports and Alcohol and Other Drug (AOD) concerns.

As the Registered Mental Health Nurse, you will:





- Undertake comprehensive clinical and biopsychosocial assessments to identify mental health, physical health, psychosocial and AOD-related needs.
- Develop, implement and review recovery-oriented care plans in collaboration with clients and multidisciplinary stakeholders.
- Coordinate care across multiple providers to ensure integrated, responsive and person-centred support.
- Provide evidence-based nursing interventions, including mental health monitoring, psychoeducation and therapeutic support.
- Undertake suicide risk assessment, safety planning and ongoing risk monitoring.
- Support medication management through clinical monitoring, adherence support and liaison with prescribing practitioners.
- Identify and respond to physical health needs, health inequities and barriers to care experienced by people living with mental illness.
- Promote preventative health care, chronic disease management and health literacy.
- Facilitate access to psychosocial, vocational, housing, community and AOD services to support holistic recovery.
- Act as a key clinical liaison for GPs, supporting shared care arrangements and coordinated treatment planning.
- Provide clinical leadership, mentoring and guidance to multidisciplinary team members.
- Contribute to clinical governance, quality improvement, audit and service development activities.

This position offers flexibility in location across the Wheatbelt, including Merredin, Moora, and Narrogin. Up to full time hours are available, with flexibility to suit the right candidate.

About You

You are an experienced mental health nurse who is passionate about integrated care and improving outcomes for people experiencing mental health challenges.

You are confident working with complexity, enjoy collaborating across disciplines and bring a strong commitment to recovery-oriented and person-centred practice.

You bring:

- Extensive experience as a Registered Nurse within a mental health setting.
- Experience undertaking comprehensive clinical and risk assessments, including suicide risk assessment and safety planning.
- Strong knowledge of care planning, care coordination and multidisciplinary models of care.
- Sound understanding of medication management and monitoring within a mental health context.




- A robust commitment to recovery-oriented, person-centred and trauma-informed practice.
- Excellent communication and stakeholder engagement skills, particularly when working with GPs and primary care providers.
- Well-developed clinical reasoning, documentation and decision-making skills.
- Experience providing clinical leadership, mentoring and support to other clinicians.

Essential Requirements

- Current unrestricted registration as a Registered Nurse with the Nursing and Midwifery Board of Australia (AHPRA).
- Current National Police Clearance.
- Current WA Driver’s Licence.

Desirable

- Postgraduate qualification in Mental Health Nursing or a related field.
- Experience working with people experiencing co-occurring mental health and AOD concerns.
- Knowledge of local community, psychosocial and vocational support services.
- Experience contributing to quality improvement and clinical governance activities.
