Tweddle is a registered public hospital with its head office located in Footscray offering a range of early parenting programs including residential, day long and sessional groups at our main campuses at Footscray and Wyndham.

Weare looking for a Family & Community Connections Coordinator four days per week at our Footscray location toprovide clinical coordination across Tweddle programs to ensure familiesreceive seamless, tailored support from referral to discharge as well as other initiatives aligned to our strategic plan.
What you'll DO.
Thisposition provides leadership to the Assessment & Intake andHome Visiting Teams, clinical advice for high risk and complex families across the service with the key objectives listed below:
- Improving referral pathways andstrengthen wrap around supports and early interventions for familiesexperiencing a period of vulnerability
- Provide substantialpost-discharge follow up options and to progress recommendations andreferrals to improve outcomes of families.
- Strengthening access toservices by building partnerships with families, communities, and externalproviders
- Collaborate with the clinicalleadership team to explore and implement creative and agile strategiesto ensure high-quality early parenting services.
What you'll bring
- A Bachelor levelqualification in Allied Health or Nursing
- Strong stakeholder engagementskills and capacity to represent Tweddle in sector forums
- Experience supporting familieswith multiple or complex issues
