Your Role

n

As a Registered Nurse / Clinical Care Partner, you'll combine clinical expertise with leadership and coordination skills to ensure clients receive high-quality, person-centered care.



You'll work closely with our Managing Directors and Care Manager to deliver exceptional support and ensure our clinical standards are met and exceeded.

Key Responsibilities:

n

Conduct client assessments and develop individualised, meaningful care plans.

n

Oversee clinical care, ensuring all care documentation is accurate and compliant.

n

Provide mentorship, supervision, and clinical guidance to Caregivers.

n

Liaise with clients, families, and health professionals to ensure continuity of care.

n

Conduct in-home clinical visits and assessments.

n

Identify opportunities to improve care processes and outcomes.

n

Participate in on-call rotation for after-hours support (shared roster)

n

Participate in on-call rotation on an as-needed basis for emergency situations and after-hours client support.

About You

n

You are a Registered Nurse who is passionate about making a difference and thrives in a collaborative, community-focused environment.

n

Current AHPRA registration as a Registered Nurse.

n

Experience developing care plans and coordinating client services.

n

Minimum 3-5 years of experience developing care plans and coordinating client services.

n

Knowledge of Aged Care Quality Standards.

n

Current CPR & First Aid, National Police Check and Working with Children Check.

n

Current Driver's License and reliable vehicle.

n

Robust communication and leadership skills, with a genuine passion for person-centered care.

n

Specific nursing experience in either aged care or home care environments is required.
