This is a permanent part-time opportunity offering rostered shifts and meaningful work supporting people to live independently, participate in their community and achieve their individual goals.

About the Role As a Community Support Worker, you will provide person-centred support based on each customer's individual needs, preferences and support plan.

n

Your responsibilities may include:

n

- Supporting customers with daily living activities and personal care
n
- Assisting with community access, appointments and social activities
n
- Providing medication assistance in line with relevant procedures
n
- Supporting customers to develop independence and practical life skills
n
- Assisting with household tasks and meal preparation
n
- Maintaining accurate case notes and documentation
n
- Following individual support plans and positive behaviour support strategies
n
- Promoting customer choice, dignity, safety and wellbeing
n
- Working collaboratively with families, carers and other professionals
n

About You

You are compassionate, reliable and committed to supporting people to achieve meaningful outcomes. You understand the importance of professional boundaries and providing support that respects each person's individual choices.

n

To be successful, you will have:

n

n
- Previous experience in disability, mental health or community services
n
- A person-centred and recovery-focused approach
n
- Strong communication and relationship-building skills
n




- The ability to work independently and respond calmly to changing situations
n
- Good written communication and documentation skills
n
- Flexibility to work rostered shifts, including mornings, afternoons, evenings, weekends and public holidays as required
n

n

Essential Requirements

n

n
- Certificate III or IV in Individual Support, Disability, Mental Health or Community Services ( or willing to work towards)
n
- Current NDIS Worker Screening Check
n
- Working with Children Check, where required
n
- Current First Aid and CPR certificates
n
- Current Australian driver's licence
n
- Access to a reliable and insured vehicle
n
- Australian working rights
n
- Ability to meet the inherent requirements of the position
n
