NSW Health's Nowra Community Health is seeking an Aboriginal Health Worker/Practitioner identified to improve access to chronic disease care for Aboriginal people across the district. You will work under a Clinical Supervisor within the Chronic Disease for Aboriginal People model, delivering integrated care, follow-up, care navigation and culturally informed support.
