Position Classification: Nurse Practitioner / Transitional Nurse Practitioner (TNP will be under a maximum of 3-year contract paid at Clinical Nurse Consultant Grade 2 whilst undertaking requirements to become an Endorsed Nurse Practitioner. On endorsement, the position holder will transition to NP Year 1).
Remuneration: $149,408.12 - $169,429.03 pa (+ superannuation + salary packaging options)

About the role
Join our innovative aged care team as an Aged Care Nurse Practitioner/Transitional Nurse Practitioner (TNP) in Bathurst. This exciting community-based role provides advanced clinical assessment, diagnosis, treatment, and care coordination for older people, helping them remain safely in their homes and communities.

With a robust focus on preventing avoidable hospital presentations and admissions, you will work closely with Emergency Department, Palliative Care, HITH, and inpatient aged care teams to deliver timely, high-quality care and improve patient outcomes across the continuum of care.
