We are seeking an experienced and passionate Team Leader - Programs to establish and lead our newly funded Tokoko Integrated Recovery Pathways Program in the Wide Bay region.

n

This is an exciting opportunity to help shape an innovative service model designed to strengthen support before, during and after residential rehabilitation. As Team Leader you will provide leadership to a multidisciplinary team delivering care coordination, counselling, peer support, cultural support, transition support and post-treatment recovery services, helping people build sustainable pathways to recovery and wellbeing.

About the Tokoko Integrated Recovery Pathways Program

n

The Tokoko Integrated Recovery Pathways Program is a newly funded initiative designed to strengthen recovery outcomes for people accessing residential alcohol and other drug treatment in the Wide Bay region.

n

Working across Bundaberg and Hervey Bay, the program enhances the existing residential rehabilitation pathway through:

n

- Pre-admission engagement and treatment readiness support
n
- Alcohol and Other Drug (AOD) care coordination and navigation
n
- Transitional accommodation and recovery pathways
n
- Post-residential recovery and relapse prevention support
n
- Harm minimisation and psycho education.
n
- Peer and lived experience support
n
- Family and community engagement
n
- Enhanced referral pathways and service integration
n
- Digital recovery support through ConnectWell
n

The program aims to improve continuity of care, strengthen recovery outcomes and help people remain connected to support before, during and after treatment.

About the Role

n

As the Team Leader - Programs,



you will provide strategic, operational and clinical leadership across the Tokoko Integrated Recovery Pathways Program.

n

You will work closely with internal teams, community partners and stakeholders to support the establishment and delivery of integrated recovery-focused services throughout Bundaberg, Hervey Bay and the wider Wide Bay region.

n

Key responsibilities include:

n

n
- Providing leadership, supervision and professional support to a multidisciplinary workforce.
n
- Promoting evidence-based, recovery-oriented and trauma-informed practice.
n
- Supporting effective risk management, care planning and continuity of care.
n
- Leading implementation and ongoing development of the program.
n
- Supporting workforce capability, quality improvement and service excellence.
n
- Building and maintaining strong relationships with health, housing, community and government stakeholders.
n
- Supporting culturally protected and inclusive service delivery.
n
- Contributing to program evaluation, reporting and sustainability activities.
n

This role is based in Bundaberg and will involve regular travel throughout the Wide Bay region.

n

What We're Looking For

n

You will be a confident and collaborative leader who is passionate about supporting people affected by alcohol and other drug challenges.

n

To succeed in this role, you will bring:

n

n
- Tertiary qualifications in Social Work, Psychology, Counselling, Nursing, Occupational Therapy, Human Services or a related discipline, and/or relevant experience.
n
- Demonstrated leadership experience within Alcohol and Other Drug (AOD),



Mental Health or related human services settings.
n
- Experience providing supervision, mentoring or leadership to multidisciplinary teams.
n
- Strong understanding of recovery-oriented, trauma-informed and person-centred practice.
n
- Experience supporting people with complex needs and managing clinical risk.
n
- Highly developed stakeholder engagement and partnership-building skills.
n
- Excellent communication, facilitation and relationship management abilities.
n
- Demonstrated commitment to culturally safe and inclusive practice.
n
- Current Australian Driver Licence.
n

Experience implementing new programs, services or funded initiatives will be highly regarded.

n
