About the role

On the road in your own car, mobile nursing service delivery to home & community care clients across Melbourne's metropolitan area. You will see clients for assessments, wound care, clinical reviews, continence and catheter care, palliative and end of life care, dementia support, NDIS complex care management, client and carer education, immunisations, and more. Depth and breadth of experience is essential, coupled with a deeply passionate, respectful, and caring holistic approach to the Home Nursing Service environment. Your experience will take you into the homes of clients with highly complex issues, that you identify and outline in documentation, problem-solve and implement strategies or action at the visit. The role is not purely prescriptive; our team is determining the plan of care at each visit.

Key responsibilities

- Chronic wound care, including complex and non-healing wounds

- Male and female catheter care (IDC and SPC)

- Continence assessments and management planning

- Chronic disease monitoring and clinical oversight within the home environment

- Palliative and end of life care, supporting clients and families with dignity and compassion

- Clinical assessments and reviews

- NDIS complex care management

- Client and carer education

- Immunisations

- Advanced critical thinking and clinical reasoning in agile environments

About you

- No less than 10 years current clinical experience as a Registered Nurse

- Advanced critical thinking and clinical reasoning

- Confident autonomous decision making in dynamic environments

- Strong problem solving across complex client presentations

- Exemplary clinical documentation, handover notes, and escalation practices

- Ability to take quick, responsive, and appropriate clinical action

- Exceptional rapport building, communication,



and client advocacy

- Current AHPRA registration, WWCC (Vic), National Criminal History Check/Police Check with no disclosable court outcomes or convictions (no greater than 3 years since date of issue)

- Accredited BLS, First Aid, Anaphylaxis (for nurse immunisers), Manual Handling, Covid training, NDIS worker orientation and screening check

- Current vaccinations as mandated for this sector

- Comprehensively insured car and current unrestricted valid driver's licence

- Recent model smart phone, printer/scanner, laptop/desktop computer

- Software and App literacy

- Australian Citizen or Permanent Resident

- Available to work minimum 3 days per week (weekday) with willingness for occasional after hours or weekends
