Job Description
Bethanie Group is seeking a Case Manager for Home Care in Western Australia.
You will coordinate services for a portfolio of clients from Claremont to Yokine, using a wellness and reablement approach to support independence and quality of life.
The role is permanent, with a hybrid schedule including in office days at our Gwelup office and client visits, plus some work-from-home days.
You will work within a collaborative team to deliver outstanding client outcomes.
