Job Description
The Justice Health and Forensic Mental Health Network invites applications for an Integrated Care Coordinator (HM1), TFT up to March ****, located in the Sydney Metro/MRRC.
The role focuses on coordinating care for adults with chronic or complex health conditions across the patient journey from reception, through custody, to community settings.
You will support continuity of care, case management, and transfer of care into the community, including post-release follow-up where required for
