The position functions as part of a multi-disciplinary team with a teamwork approach to the provision of low intensity therapy home-based programs to frail elderly clients who are ready for discharge from sub-acute hospital care but are unable to return home without additional care and support.
Key responsibilities
As the Physiotherapist in ITACS you will provide high quality Physiotherapy services to clients and carers, to ensure the provision of optimal health outcomes for the clients.
You will be required to have problem-solving skills to support a client’s protected mobility in the home and community setting without direct clinical supervision when completing in-home assessments and interventions.
You will also provide care planning, case management and liaise with external stakeholders.
