We’re seeking experienced and empathetic professionals to deliver high-quality, person-centred support needs assessments for people living with disability.

In this role, you will gather and analyse information, interpret evidence, and prepare clear, accurate assessment reports to support informed decision-making. You’ll communicate respectfully with participants, families and support networks, using trauma-informed and culturally-secure approaches to ensure people feel heard, understood and supported.
Key Responsibilities will include: /n

Conduct support needs assessments through guided conversations and information gathering.
/n
Analyse and synthesise participant information and external evidence to support evidence-based outcomes.
/n
Prepare clear, accurate and defensible written assessment reports.
/n
Apply policy, legislation and procedures using sound judgement and ethical practice.
/n
Communicate assessment processes and outcomes clearly across virtual and face-to-face settings.
/n
Escalate complex or high-risk matters appropriately and contribute to continuous service improvement.
/n
Collaborate across teams to support coordinated and inclusive participant outcomes.
/n
Maintain accurate records using digital systems while ensuring privacy and confidentiality.
/n
About You /n
You are a compassionate and adaptable professional with strong assessment, interviewing and stakeholder engagement skills. You can confidently manage complex information, apply sound judgement, and communicate effectively with diverse audiences.
You will demonstrate: /n
/n
Experience conducting support needs assessments or similar person-centred assessments.
/n
Solid analytical, report writing and decision-making capability.
/n
Commitment to inclusive, accessible and culturally-protected practice.
/n
Trauma-informed and strengths-based approaches to participant engagement.
/n
Ability to manage competing priorities in a fast-paced environment.
/n
High levels of professionalism, integrity and ethical practice.
