$108,680-$114,608
per year plus superannuation and
5 weeks of annual leave.
Your Role
As a
Registered Nurse / Clinical Care Partner
, you'll combine clinical expertise with leadership and coordination skills to ensure clients receive high-quality, person-centered care.



You'll work closely with our Managing Directors and Care Manager to deliver exceptional support and ensure our clinical standards are met and exceeded.
Key Responsibilities:
Conduct client assessments and develop individualised, meaningful care plans.
Oversee clinical care, ensuring all care documentation is accurate and compliant.
Provide mentorship, supervision, and clinical guidance to Caregivers.
Liaise with clients, families, and health professionals to ensure continuity of care.
Conduct in-home clinical visits and assessments.
Identify opportunities to improve care processes and outcomes.
Participate in on-call rotation for after-hours support (shared roster)
Participate in on-call rotation on an as-needed basis for emergency situations and after-hours client support.
About You
You are a Registered Nurse who is passionate about making a difference and thrives in a collaborative, community-focused environment.
Current
AHPRA registration
as a Registered Nurse.
Experience developing care plans and coordinating client services.
Minimum 3-5 years of experience developing care plans and coordinating client services.
Knowledge of
Aged Care Quality Standards
.
Current
CPR & First Aid
,
National Police Check and
Working with Children Check.
Current
Driver's License
and reliable vehicle.
Strong communication and leadership skills, with a genuine passion for person-centered care.
Specific nursing experience in either aged care or home care environments is required.
