The Illawarra Shoalhaven Local Health District is seeking a Physiotherapist to join the ITACS team, delivering low intensity, home-based therapy programs for frail elderly clients preparing for discharge from sub-acute hospital care.
You will assess and treat clients in their homes, support protected mobility, develop care plans, and liaise with carers and external stakeholders to coordinate in-home services and smooth transitions back to the community.
