We're seeking a passionate
Immediate Housing Response (IHR) Case Manager
to join our Housing & Homelessness team. In this rewarding role, you'll support individuals, couples, and families experiencing homelessness or housing instability to access safe, sustainable housing through case management, advocacy, and practical support.

About the Role
As an IHR Case Manager, you will provide intensive short to medium-term case management support to people experiencing homelessness or housing instability. You will be working across our Redcliffe and Lawnton sites.
Working within a strengths-based, trauma-informed, and person centred framework, you will:
Manage a caseload of individuals, couples, and families residing in temporary accommodation.
Conduct assessments, develop support plans, and coordinate referrals.
Support clients to achieve safe and sustainable housing outcomes.
Advocate with housing providers, government agencies, and community services.
Assist clients to navigate housing systems, applications, and tenancy requirements.
Support tenancy readiness and tenancy sustainment activities.
Build strong relationships with housing and accommodation providers, and community stakeholders.
Maintain accurate case records, reporting, and client documentation.
Contribute to collaborative responses that improve housing outcomes.

About You




You are passionate about helping people overcome barriers to housing and achieve greater stability and independence.
You will bring:
A degree qualification in Social Work, Human Services, or a related discipline, or equivalent experience.
Experience supporting people experiencing homelessness, housing instability, or complex needs.
Strong assessment, case management, advocacy, and support planning skills.
Knowledge of housing pathways, homelessness services, and tenancy support.
Experience building effective relationships with housing providers, government agencies, and community organisations.
Strong communication, stakeholder engagement, and problem-solving skills.
Experience maintaining case records, reporting requirements, and client management systems.
A commitment to strengths-based, trauma-informed, culturally safe, and person-centred practice.
Mandatory Requirements
Current Queensland Driver Licence.
Current Blue Card.
Current National Police Check.
Full Australian working rights.
