About the role
We're hiring a Care Liaison Manager to own the relationship between Prevail Care and the NDIS ecosystem that surrounds our participants — Support Coordinators, Plan Managers, LACs, NDIA planners, allied health, hospital discharge teams, guardians, and families.
This is not an entry-level coordination role. We want someone who already knows the Sydney NDIS landscape and walks in with working relationships — someone Support Coordinators already trust, who knows which hospital social workers to call, and who can bring referrals through the door because people already know their name. You'll manage intake and onboarding for new participants, keep existing supports running smoothly, and act as the primary point of contact for every external stakeholder involved in a participant's plan.
The role directly affects our referral pipeline, participant outcomes, and our standing with the Support Coordination community. It sits close to leadership and carries real authority.
What you'll do
Build and maintain referral relationships with Support Coordinators, Plan Managers, LACs, hospital discharge planners, allied health providers, and community organisations across Sydney
Manage the intake pipeline end to end:



initial enquiry, needs assessment, plan review, SIL quoting and roster of care, service agreements, and onboarding
Develop person-centred support plans in collaboration with participants, families, guardians, and multidisciplinary teams, aligned to NDIS plan goals and funded supports
Act as the primary liaison for external stakeholders — attend case conferences, plan reviews, hospital discharge meetings, and NDIA-facing discussions on behalf of the organisation
Advocate for participants' rights, preferences, and best interests, including through plan reviews and change-of-circumstances requests
Monitor service delivery against NDIS Practice Standards and the NDIS Code of Conduct, identifying issues early and resolving them
Maintain accurate, audit-ready participant documentation in line with NDIS Quality and Safeguards Commission requirements
Work with our rostering and operations team to ensure supports are staffed, delivered, and claimed correctly against plan budgets
Contribute to continuous improvement of intake, onboarding, and stakeholder communication processes
What we're looking for
An established professional network within the NSW NDIS sector — Support Coordinators, Plan Managers, allied health, hospital and justice liaison contacts — and the ability to demonstrate it




Proven experience in NDIS intake, support coordination, case management, or business development for a registered provider
Strong working knowledge of the NDIS: plan structures, funded support categories, pricing arrangements and price limits, SIL and SDA, and the NDIA planning and review process
Familiarity with the NDIS Practice Standards, NDIS Code of Conduct, and Quality and Safeguards Commission expectations, including incident and restrictive practice reporting
Experience working with participants with complex needs — psychosocial disability, forensic or justice involvement, acquired brain injury, or high-intensity supports — is highly regarded
Excellent communication and relationship skills; comfortable representing the organisation with clinicians, government, and families
Ability to manage a caseload and an intake pipeline concurrently, with strong organisation and follow-through
Proficiency with NDIS-focused care management software and Microsoft Office
Certificate IV in Disability, Community Services, or equivalent qualification; tertiary qualification in social work, nursing, allied health, or a related discipline is an advantage
Current NDIS Worker Screening Check and Working With Children Check (or willingness to obtain prior to commencement)
Valid driver's licence and access to a vehicle for participant and stakeholder visits
