The Chance

Key Responsibilities

- Providenursingandpersonalcareservicestoclientsintheirhomes
- Deliverhigh-quality,person-centredcarewhilepromotingindependenceandwellbeing
- Maintainaccuratecaredocumentationandclientrecordsusingdigitalsystems
- Developandfollowindividualcareplans
- Communicateeffectivelywithclients,families,andmultidisciplinaryhealthcareteams
- Monitorclienthealthandreportchangeswhenrequired
- Maintainsafeworkpracticesandcomplywithorganisationalpolicies
- Providecompassionate,respectful,andprofessionalcareatalltimes

Requirements & Eligibility

- CurrentregistrationasaRegisteredNurseorEnrolledNurse(whereapplicable)
- Experienceinhomecare,communitynursing,disability,oragedcarepreferredbutnotessential
- Currentdriver'slicenceandaccesstoareliablevehicle
- CurrentNationalPoliceClearance
- CurrentFirstAidandCPRcertification
- Strongcommunicationandinterpersonalskills




- Confidentcomputerskillsforelectronicdocumentationandcommunication
- Commitmenttodeliveringhigh-quality,client-focusedcare
