Life Without Barriers is seeking a Case Manager – Family Based Care in Goulburn/Melbourne to coordinate care, safety and wellbeing for children in out-of-home care. You will lead case planning, access education, health and therapeutic services, and advocate for the child’s best interests.

You will collaborate with families, carers, and government agencies, manage complex caseloads, and support permanency planning and transition to independence as appropriate.
