Vision Australia is seeking a phone-based Client Service Navigator – Aged Care to provide specialist service navigation and end-to-end coordination for aged care funding including Support at Home and Commonwealth Home Support Program (CHSP).

You will take ownership of the client journey from referral through to service commencement, acting as the single point of contact for Support at Home stakeholders and advising Program Managers on funding options for Vision Australia equipment and services.
