Job Description

Medibank in Adelaide is seeking a part time Placement Support Officer (0.8 FTE) to provide essential operational and general support to the Social Work team. You will help patients transition from hospital care into suitable residential aged care or community placements with a focus on protected, timely and person-centred discharge outcomes.

Based in Adelaide's CBD and Walkerville, you will identify, coordinate and support transition planning under the Transition Care Services framework.
