About the role

Make a real difference. Join HOMHS, a collaborative outreach service supporting people experiencing mental illness and homelessness to achieve secure housing, improved wellbeing and lasting recovery through integrated, person-centred care.

- Develop and implement client-directed, strengths-based individual client service packages from entry to exit
- Work collaboratively with clients and the HOMHS team to conduct comprehensive assessments, identifying needs, aspirations and goals of clients
- Provide case management support and advocacy to consumers with a range of complex needs who are accessing HOMHS
- Conduct risk assessments and manage risks responsibly and in line with program policy

- Relevant tertiary qualification and/or demonstrated experience in mental health, AOD, homelessness or related field
- Demonstrated experience providing client-centred, strengths-based case management from intake to exit
- Strong knowledge of mental health conditions, treatment interventions, AOD practice, harm minimisation principles, and the interface between community mental health, homelessness and broader health service systems
- Ability to deliver recovery-oriented services that promote client choice and self-determination

All appointments are made subject to a satisfactory Nationally Coordinated Criminal History Check (NCCHC), a Working with Children Check (WWCC), complete a Pre-Employment Declaration Form, evidence of legal rights to work in Australia, and evidence of vaccination.

Successful applicants will be required to provide evidence of immunisation assessment including Influenza within 12 weeks of commencement (B). This is in accordance with the Victorian Minister of Health Mandatory Vaccination Orders and legislated requirement for influenza vaccination.
