About the role

n

HITH/PAC Home Nurse - Registered Nurse

n

Permanent - 0.4 EFT

n

TheHITH/PACHome Nurse rolewillbe responsible forassessing, planning, delivery and evaluating safe andappropriate consumercentred nursing care in the community setting. The nurse can assess,treatand escalation appropriately while managing the care of multiple clients independently, working collaboratively with the Home Nursing team in the provision of complex care in the community setting. The nurse ensuresthat the delivery of quality, evidence based best practice of care is delivered, whilebeinga leader and mentor to junior staff and students within theHomeNursing team.

The HITH/PACHome Nursewill act as a clinical resource within the teamandprovide clinicalexpertiseandproblem-solvingability for service provision.

What's Required?

n

Registered Nurse Division 1 holding current registration with the AHPRA (Australian Health Practitioners Regulation Agency),



with a minimum of2 yearpost qualification clinicalexperience

n

BasicLifeSupport, Anaphylaxis, CVAD, Venepuncture and IV cannulation competencies

n

Demonstrated competency in clinical skills relevant to home nursing,including HITH/PACclinicalcare,complex wound management,continenceanddiabetesmanagement andpalliativecare

n

Ability to perform comprehensive clinical assessments and develop, implement,and evaluate goal-directed care plans

n

Proficient in safe medication management and complex care procedures

n

Ability to practice independently, set priorities, and manage a clinical workload in a home setting, while also contributing totheteam

n

Highly developed verbal and written communication skills for patient education, documentation, and liaison with stakeholders

n

Demonstrated commitment to patient-centred care, quality improvement, and safety standards

n

Understanding of medico-legal documentation compliance and competence inusingelectronic documentation systems(TRAK)for patient records and data collection

n

High levelinterpersonal and professional organisationalskills

n

Knowledge and understanding of government legislation, policies and procedures risk management and OH&SrelativetoHomeNursing within the Public sector

n

Current Victorian DriversLicence,Working withChildren Check, and clear Police Check

n
