Job Description

Align Care in Canberra is seeking a compassionate Carer / Disability Support Worker to assist with personal care, daily living tasks, meal preparation and light household duties.

You will support community access, accompany appointments, and provide emotional support and companionship per care plans.

Diligent record-keeping, progress notes, and collaboration with participants, families and staff are essential to promote independence and wellbeing.
