Job Description

Hi Five Community Services is seeking a qualified Community Care Registered Nurse who speaks Hindi and Punjabi to join our team. The role involves delivering high-quality clinical support to a diverse client base, including clients who primarily communicate in Hindi and Punjabi, while ensuring protected, accurate, and culturally appropriate care delivery.

Key Responsibilities

- Provide direct nursing care and clinical support to clients in home and community settings.
n
- Conduct client assessments, care planning, and clinical documentation in line with regulatory standards.
n
- Communicate clinical information clearly and accurately with clients and families, including those who speak Hindi and Punjabi as their primary language.
n
- Liaise with allied health professionals, support coordinators, and family members.
n
- Ensure compliance with relevant clinical governance, medication management, and infection control standards.
n
- Maintain accurate and timely clinical records.
n
- Train and upskill support workers in clinical care delivery for clients with high or complex care needs.

Skill & Experience

n

n
- Current registration as a Registered Nurse with AHPRA.
n
- Relevant nursing qualification (Bachelor of Nursing or equivalent).
n
- Demonstrated experience in community, disability, or aged care nursing.
n
- Current National Police Check and NDIS Worker Screening Check (or willingness to obtain).
n
- Current First Aid and CPR certifications.
n
- Valid driver's license and own vehicle (if applicable).
n
- Experience working within NDIS or aged care service delivery.
n
- Experience in culturally responsive care planning.
n
- Ability to communicate in Hindi and Punjabi is highly desirable.

Salary & Employment Conditions

- Salary: $90,000 per annum + superannuation
n
- Employment type: Full-time, ongoing
