Workforce Extensions Sunshine Health is seeking a compassionate and reliable Care Partner for our client, Solara Health. If you have experience supporting older people in their homes and are passionate about providing high-quality, person-centered support, we'd love to hear from you.

What do you need?

n

n
- Qualifications or experience in Nursing is preferred
n
- Experience in Case Management is preferred
n
- Robust time management and organisational skills
n
- Excellent verbal and written communication
n
- Ability to build positive relationships with clients, families, and advocates
n
- Ability to work autonomously and in a team environment
n
- Current Police Check, driver's licence
n

Duties & Resposibilities

n

n
- Provide end-to-end support for clients receiving Support at Home Packages, including assessment and commencement
n




- Work with clients to identify strengths and abilities and deliver appropriate care and services
n
- Develop, review, and update care plans to support client goals
n
- Liaise with medical, allied health, and community stakeholders
n
- Maintain regular client contact, including monthly monitoring calls
n
- Implement and manage client budgets
n
- Ensure compliance with Aged Care Quality Standards
n
- Speaking a second language is an advantage
n
- Vehicle provided/own: company vehicle provided
n
- Pay rates: $80K + super
n
- Reporting to: Community Care Manager
n
