Job Description

ForHealth is seeking a CDM Nurse (RN or EEN) to join our Ballarat centre. You will deliver chronic disease management and preventative care, working closely with GPs and the healthcare team to support patients with complex conditions. This role is based in Ballarat, VIC, and offers a secure day schedule with no night shifts.

The role focuses on care planning, health assessments, and ongoing coaching to improve long-term health outcomes, with autonomy and impact in the community.
