Job Description
Berry Street is seeking a Case Manager for the GOALS+ housing and support program, based across Morwell and Traralgon. The role focuses on strengths-based, trauma-informed case management and practical support for young people aged * leaving care or experiencing homelessness.

You will develop individualized plans, connect participants to housing, education and health services, and help build life skills to enable greater independence.
